This job is currently a work from home position, however, this can change at any time to come back into the office. Applicant must be able to train in Danville for approximately one month and be able to easily commute to one of the PATHS offices.

Job Description:

  1. Provides orientation to patients and conducts initial psychosocial and financial assessments as needed. 
  2. Provides patients with appropriate resources within PATHS and to other organizations.
  3. Serves as resource contact for all patients, particularly those who live in rural areas and in underserved populations.
  4. Works collaboratively with and takes day-to-day instruction from the direct supervisor.
  5. Builds relationships with community network and screening programs to increase access to patient care.
  6. Proactively and assertively addresses issues as they arise.
  7. Performs other duties as assigned. 

 

Preferred Skills:

Education:  BSW or BA/BS in Social Work or Social/Human Services, prefered.  Minimum of three years experience in human/social services, preferably in a healthcare environment, preferred.

Qualifications:  Must possess a valid driver’s license and reliable transportation.  Candidates must demonstrate excellent communication and relationship skills, have diplomacy, tact and a positive approach, and the willingness and ability to evaluate several sides of an issue.  Organizational skills and the ability to maintain confidential information is a must.  Candidates must be able to function independently, be flexible and able to relate to ill, disabled, elderly and emotionally upset internal and external consumers.  The staff member must be able to demonstrate the knowledge and skill set necessary to provide care appropriate to the age and mental capacities of the clients served.  Some experience in entering and processing information in a personal computer is helpful.
